The same carriageway was shut just 24 hours earlier at junctions 8 and 8a, after a refuse lorry collided with a Lexus.

A female passenger inside the Lexus was rushed to the John Radcliffe Hospital in Oxford suffering from serious and life-threatening head and facial injuries.

Another accident occurred at junction 6 the day before, when after a car drove the wrong way along a motorway for up to five miles and caused a fatal pile-up.

The Subaru Forester was seen towing a caravan south on the northbound M40 in Oxfordshire between Lewknor and Milton Common.

But it collided with a Ford Mondeo and a Ford Focus – killing the Subaru driver and passenger, both in their 80s, and the Mondeo driver, a man in his 30s.
